We offer dedicated services for youth, focusing on prevention and education to promote healthy decision-making and reduce risky behaviors. Through mentorship, support for teen moms, life skills training, and substance abuse prevention, we aim to equip youth with the tools they need to succeed. We’re here to meet the diverse needs of youth, helping them build confidence, stay safe, and thrive in every aspect of their lives!

TEEN MOM'S PROGRAM

Our Teen Mom’s Program,, Unidos Formando Un Futuro Program is a three-year program which supports young expecting moms up to the age of 30 years old either in their first and second pregnancy or who already have a new born who is not older than three months, living within the Little Village, Pilsen, Back of the Yards and West Lawn areas. Recognized as one of Illinois’ highest-quality programs, we are committed to empowering young moms for a brighter future. Parent Groups offer additional support, enhancing the impact of home visits based on the Healthy Families America model, to improve wellness for both parent and child.

THE HOME VISITING PROGRAM

The Home Visiting Program supports young mothers with personalized, in-home visits based on the Healthy Families America model, for young expecting moms with the same eligibility criteria as our Unidos Formando Un Futuro Program. These visits provide hands-on guidance and access to resources, helping young parents build essential skills in a comfortable setting to succeed as parents. Serving young moms living within the Little Village, Pilsen, Back of the Yards and West Lawn areas; this program helps young moms build strong, nurturing parent-child relationships, gain self-sufficiency, and access child development screenings and childcare support.

REIMAGINE YOUTH DEVELOPMENT SERVICES

The Reimagine Youth Development Services program is here to support youth aged 11 to 17 at risk of exposure to gun violence, in the Gage Park and South Chicago communities. With weekly academic tutoring, including engaging STEM-based lessons, Like Skills training and Civic Engagement and Leadership Development curriculum, this program empowers young minds to thrive. Program specialists bring life skills education and organize sports and recreation activities to keep participants active and engaged.

MENTORING PROGRAM

Our Mentoring Program provides at-risk youth with a brighter path. This program offers at-risk teens in communities affected by violence a dedicated mentor, guidance on conflict resolution, trauma-focused support, and curriculum-driven mentorship to help them stay in school, graduate, and embrace positive choices.

YOUTH RISK PREVENTION CENTER PROGRAM

A Youth Risk Prevention Center Program provides support and resources to at-risk youth, helping them avoid harmful behaviors like substance use, violence, and school dropout. The program offers counseling, group support, life skills training, and substance use prevention to guide young people toward healthier choices and positive outcomes. It also provides educational support, helping youth stay engaged in school and develop essential skills for success.

The program creates a safe space where youth can feel secure and supported, reducing the risk of violence and risky behaviors. By fostering a positive environment and offering family and community engagement, the Youth Risk Center empowers young people to make better decisions, build resilience, and thrive in their personal and academic lives.
